California Assembly Bill 2079 Disclosures
Pursuant to California Assembly Bill 2079, commencing January 1, 2012, California postsecondary educational institutions that offer athletic scholarships shall provide all of the following information on their Internet Web site:
ATHLETIC SCHOLARSHIPS INFORMATION
- The NCAA's policy on scholarship duration...NCAA Financial Aid Period of Award Information
- The most recent cost of attendance expenses as published by the postsecondary educational institution's financial aid office for the academic year and for the summer year... Estimated Total Costs 2022-23

- The following information relating to NCAA scholarship rules: "Pursuant to NCAA rules, a verbal commitment is not binding on either the student-athlete or the institution. The National Letter of Intent is a binding agreement between a prospective student-athlete and an institution in which the institution agrees to provide a prospective student-athlete who is admitted to the institution and is eligible for financial aid under NCAA rules athletics aid for one academic year in exchange for the prospective student-athlete's agreement to attend the institution for one academic year. The National Letter of Intent must be accompanied by an institutional financial aid agreement. If the prospective student-athlete signs the National Letter of Intent but does not enroll at that institution for a full academic year, he or she may be subject to specific penalties, including loss of a season of eligibility and a mandatory residence requirement."

ATHLETICALLY RELATED MEDICAL EXPENSES INFORMATION

Insurance Coverage and Medical Payment Policy The athletic department maintains secondary medical insurance coverage on student-athletes through the CSURMA AIME program which is managed by HSR. Therefore, all student-athletes who are covered by a family or personal policy will utilize that insurance first when making payment of hospital and medical charges. Any expenses that are greater than those covered by an applicable insurance policy will be cover under the program listed above if injury occurs during a supervised practice, games or training session. In order for a student-athlete to be eligible, you must be registered for the appropriate intercollegiate sport’s class. Medical treatment must be provided by an approved medical provider that can provide itemized medical billing or claim codes in order to process insurance payment. In the event that these requirements have not been met or the provider has not be authorized, claims will be the sole responsibility of the student-athlete.

Student-athletes who do not have medical insurance coverage will be covered by the CSURMA AIME program for athletic related injuries ONLY. In any non-emergent situation, all doctors, hospitals, and other medical details, must be cleared through the athletic training office before proceeding. Medical treatment must be provided by an approved medical provider that can provide itemized medical billing or claim codes in order to process insurance payment. In the event that these requirements have not been met or the provider has not be authorized, claims will be the sole responsibility of the student-athlete.

TRANSFER INFORMATION
Effective immediately for student-athletes transferring for the 2022-23 academic year. The NCAA policy includes the following changes:
- Requires undergraduate student-athletes to complete an educational module related to transferring before the school may enter the student-athlete's information into the NCAA Transfer Portal.
- Eliminates the previous school's ability to object to the use of the one-time transfer exception.
- Requires the head coach of the certifying school and the student-athlete to certify in writing that no athletics staff member or other representative of the school's athletics interest contacted the student-athlete or any individual associated with the student-athlete (e.g., family member, scholastic or non-scholastic coach, advisor), directly or indirectly, without first obtaining authorization through the notification of transfer process.
- Establishes June 15 as the date by which a student-athlete must provide written notification of transfer to the school to use the one-time transfer exception (not applicable to midyear transfers).
- Permits schools to reduce or cancel an athletics aid agreement previously signed for the next academic year if a student-athlete requests to be entered into the Transfer Portal. It would remain impermissible to reduce or cancel a student-athlete's athletics aid during the academic year in which the student-athlete provides notification.
The CPP Department of Intercollegiate Athletics only accepts written notification of transfer via this form, which must be submitted to the Compliance Office for your information in the NCAA Transfer Portal. Written or emailed requests will not be accepted and will not trigger the window for entry into the NCAA Transfer Portal.
