INTRODUCTION

Cal Poly Pomona athletic trainers, under the supervision of the team physician, and in collaboration with other health care professionals, provide preventative services, emergency care, clinical diagnosis, therapeutic intervention and rehabilitation for injuries and medical conditions.

PRE-PARTICIPATION STEPS FOR 2026-27

STUDENT-ATHLETE MEDICAL ELIGIBILITY CENTER

This page was created to assist new incoming student-athletes in the initial process of satisfying medical eligibility requirements set forth by the NCAA and Cal Poly Pomona Athletics. It is the responsibility of the student-athlete to complete all relevant medical history information and be aware of the nature of the services that will be provided once medical eligibility requirements have been satisfied.

INTRODUCTION TO THE PRE-PARTICIPATION EVALUATION (PPE)

The Cal Poly Pomona Sports Medicine pre-participation medical evaluation process includes the following components. Each component must be completed prior to satisfying medical eligibility. Incomplete medical evaluations will result in medical ineligibility and the student athlete will not be allowed to practice or compete until this evaluation has been completed. Each component to the pre-participation evaluation has been outlined below.
 
  • NEW STUDENT-ATHLETES (FRESHMEN & TRANSFERS) - COMPLETE STEPS 1-3
    • Step 1 – NCAA Mandatory Sickle Cell Reporting - You are required to submit your sickle cell test results as part of your medical clearance. If you have not been tested for sickle cell or do not have your results, you can request a sickle test with your physician or contact CPP Student Health Services (once you have been admitted to the university and have your Bronco ID number) at 909-869-4000 to schedule a test. Fees may apply. 
    • Step 2 – Print and complete New Student-Athlete Pre-participation Physical Form.
    • Step 3 – You MUST use the New Student-Athlete Pre-participation Physical Form when seeing the physician for your exam. The completed form MUST be signed by an MD (Medical Doctor) or DO (Doctor of Osteopathic Medicine) for medical clearance. If you have been admitted to the university and have your Bronco ID number, you have the option to make an appointment through Cal Poly Pomona Student Health Services by calling 909-869-4000 to schedule your physical exam. Fees may apply.
      • Your physical exam paperwork and sickle cell results MUST be submitted to the CPP Sports Medicine Staff before you can participate in any countable athletic related activity.
      • Your physical exam paperwork and sickle cell results MUST be submitted prior to receiving any medical care from Sports Medicine Staff or using any facilities related to athletics.
 
  • RETURNING STUDENT-ATHLETES - COMPLETE STEPS 1 & 2
    • Step 1 – Print and complete Returning Student-Athlete Pre-participation Physical Form.
    • Step 2 – You MUST use the Returning Student-Athlete Pre-participation Physical Form when seeing the physician for your exam. The completed form MUST be signed by an MD (Medical Doctor) or DO (Doctor of Osteopathic Medicine) for medical clearance if your exam is being completed OFF-CAMPUS. You also have the option to make an appointment through Cal Poly Pomona Student Health Services by calling 909-869-4000 to schedule your physical exam. 
      • Your physical exam paperwork MUST be submitted to the CPP Sports Medicine Staff before you can participate in any countable athletic related activity.
